Surgical Booking Forms
Patients scheduled for surgery are required to complete some or all of the forms below. The forms do not need to be printed. They can be downloaded and completed electronically in any program that supports PDF documents. You may require PDF-viewing software such as Adobe Acrobat Reader for Windows or Mac, Adobe Fill & Sign for iOS or Adobe Fill & Sign for Android. Please contact your urologist for directions on which forms are required for your surgery or procedure.
You may return completed forms by e-mail or through our Medeo Secure Messaging portal. If you choose to return completed forms by email, please be aware that e-mail is not considered a secure form of communication. We cannot guarantee the security and privacy of any Personal Health Information sent by e-mail. By returning your form using e-mail, you acknowledge that your Personal Health Information carries an inherent risk of disclosure to third parties.
